The following is a tutorial on Adobe Photoshop. It covers how to create a bullet hole effect. In addition, it shows how to add the effect as a brush stroke, so you can easily add more of these effects.
Start a new document as shown below. (File -> New from the menu)

From the Toolbar, choose the Brush tool.

From the menu select a smaller diameter for the brush, say size 5.

Click the brush options icon over to the right along the top menu.

Under brush tips, set the Size Jitter to 100.

Create a new layer using the Add New Layer button at the bottom of the layers palette.

While holding down the shift button test using the brush from top to bottom of your workspace.

From the menu choose Edit -> Define Brush Preset...

Choose a name for your new brush and click the OK button to save.

You can also improve the brush further by using the following settings...
Under Shape Dynamics again, set the Size Jitter, Angle Jitter and Roundness Jitter all to 100 this time.

Also under Brush Tip Shape...

Set the Spacing to 80%.

Clicking on you workspace, and slowly moving around in a small circle produces the following effect.

Clicking the left square bracket decreases the size of the brush. After a few size decreases, you create a spiral effect from the centre outwards. This gives the effect of small sideways cracks in the glass. Note that this may be performed a few times until you are happy with the look.

Now choose the gradient tool from the toolbar.

Choose the radial gradient button.

From the gradient toolbar click it once...

Choose the Foreground to Transparent option as shown below...

Now with this gradient radial tool chosen, draw a small line from the centre to about halfway outwards. The effect here is to produce a little more black in the centre.

From the toolbar, choose the eraser tool.

Set a small scatter brush as shown below.

Simply click in the centre a few times to produce the actual bullet hole as shown below.

Again, from the menu choose Edit -> Define Brush Preset... and save your new brush.

From the brush dropdown list located at the top of the screen, select you new brush.

On any new workspace, you may use the brush to create as many new bullet holes as you need... quickly and easily!
